HGV/PSV Provisional Licence
To obtain an HGV/PSV Provisional Licence you need
two forms - D2 and D4 available from Alex McAra HGV/PSV
Training.
D2 Licence Application Form
Make sure you apply for the correct provisional
entitlement as follows:
For HGV tick the category "C" box.
For PSV tick the category “D and or D1” for PSV.
Complete all further details required.
Photo-Card Licence Application Form.
You must apply for a photo-card licence at the time
of your vocational application.
(Unless you currently hold a photo-card licence).
D4 Medical Report Form
Contact your Doctor and arrange an HGV/PSV Medical.
Take form D4 with you.
This will cost approximately £140.
We can offer a more cost effective Medical at a
Perth Medical Centre for £85.00 plus VAT or for
£60.00 plus VAT at a Dundee Medical Centre - Contact
us for further details.
When posting completed forms allow 3-4 weeks for
your licence to be returned.
In the interim period you should purchase the study
books and DVD for your computer tests which must be
sat and passed before sitting your driving test.

THE OFFICIAL DSA THEORY TEST FOR DRIVERS OF LARGE
VEHICLES (make sure the book is the 2012 edition).
THE OFFICIAL DSA GUIDE TO DRIVING GOODS VEHICLES.
THE OFFICIAL DSA GUIDE TO HAZARD PERCEPTION.
PSV Courses
THE OFFICIAL DSA THEORY TEST FOR DRIVERS OF LARGE
VEHICLES (make sure the book is the 2012 edition).
THE OFFICIAL DSA GUIDE TO DRIVING BUSES AND COACHES
THE OFFICIAL DSA GUIDE TO HAZARD PERCEPTION.
These titles are available from most good book shops or
can be purchased on-line from our "Further
Reading section" or are available from Alex
McAra HGV/PSV Training.
